The first, his actual health status, let’s put aside for the moment.
The second, that he’s losing, is out in public.
Frankly I think “is losing” is kind of a category error. The last 15 years or so have lulled us into thinking “if the election were held today” is a meaningful thing we can talk knowledgably about, and I don’t think it is or we can.
