It should be zero, and if they're using material that causes burns then epecialised safety equipment needs to be on-site. But this is the US, thus...

No industry has a zero worker injury rate. Nor a zero percent citation rate. Absolute numbers aren't useful. Comparative numbers are.

Seriously, just google terms like "worker dies in tunnel". An article that doesn't give any sort of comparative sense as to how common it is at one company vs. the industry in general is just rage-clickbaiting.

That’s a solidified intestine with serious issues.
