Depressed mental status
Decreased respiratory rate
Decreased tidal volume (shallow breathing)
Miotic (constricted) pupils
Nonspecific symptoms like lightheadedness, dizziness, palpitations, numbness & tingling, fear, anxiety, etc. are not indicative of opioid toxicity.
